Few treatments earn a place on as many different skin care plans as microneedling. It addresses everything from acne scars to pore texture, is safe across skin tones, and treatments fit easily between injectable appointments. And unlike many skin resurfacing options, it doesn’t require a week out of your life to heal.

For patients throughout the Charleston area, microneedling is often the treatment that fills the gap between a solid at-home skincare routine and the results they actually want. 

Its popularity also comes with questions, like how to use it safely and how to get the most from each session. Below, our Mount Pleasant med spa team answers the ones that come up most often.

1. Can microneedling help with acne scars?

Yes, acne scars are one of the most well-researched applications for microneedling. SkinPen works by creating micro-channels in the skin that trigger your body’s natural wound-healing response, which then stimulates the production of new collagen and elastin. Over a series of sessions, that remodeling process gradually softens the texture of atrophic (pitted) acne scars.

A three-treatment series spaced four to six weeks apart is a standard starting point, and collagen remodeling continues to improve results for weeks after the final session. For deeper or more stubborn scarring, we may discuss pairing SkinPen with laser skin resurfacing at a later stage.

2. Will microneedling make my pores smaller?

Microneedling won’t change the size of your pores (since pore size is largely determined by genetics), but it can make them appear smaller. As collagen production increases and skin texture improves, pores that look enlarged due to laxity or congestion often become less noticeable.

For patients dealing with both skin texture issues and pore congestion, we sometimes recommend a DiamondGlow facial one week before your first SkinPen session to clear the skin’s surface before treatment begins.

3. Can I get microneedling if I have active acne?

You should not receive microneedling treatment while you have active breakouts. Treating skin with open, inflamed lesions risks spreading bacteria across the face through the micro-channels the device creates—which can actually worsen your skin rather than improve it.

Once your breakouts have cleared, microneedling can be an excellent long-term strategy. Research suggests it may help moderate sebum production over time, which plays a role in reducing future breakouts. If you have ongoing acne concerns, let us know during your consultation! 4. Can I use retinol after microneedling?

No—hold off for at least one week. After microneedling, your skin’s barrier is temporarily compromised and far more absorbent than usual. Applying retinol too soon (while channels are still open and skin is sensitive) can cause redness, irritation, and peeling beyond what’s expected from the treatment itself.

For the first 24 to 48 hours, stick to a basic skincare regimen; no more than a gentle cleanser, fragrance-free moisturizer, and broad-spectrum mineral SPF. Around day three, you can resume most of your regular skincare. Retinol and other active ingredients like glycolic acid and vitamin C are best reintroduced gradually after the seven-day mark, or when your skin feels fully back to baseline.

5. How soon before a big event should I schedule microneedling?

Plan for at least two to three weeks before any event where you want your skin to look its best. Most patients see some initial redness and a mild sandpaper texture for one to three days post-treatment, and some experience light flaking around day two or three as the skin sheds. By day three to four, most patients are comfortable in public—some even the next morning with a little tinted SPF.

That said, the glow from a single session continues to develop as new collagen forms, so booking two to three weeks out gives your skin time to settle beautifully. If you’re new to microneedling and considering it before a major event, it’s worth having your first session further in advance—four to six weeks, at least—so you know how your skin responds.

6. How long after Botox or fillers can I get microneedling?

Wait at least two weeks after neurotoxin treatments (i.e. Botox & Daxxify), and two to four weeks after filler. Microneedling near recently injected areas carries a risk of displacing product before it has fully integrated.

If you see Malissa or Rebecca for injectables and our aesthetician team for skin treatments, we will coordinate your timing during the planning process. The reverse question also applies: if you’re planning Botox or fillers after microneedling, wait until your skin has fully healed—around two weeks out is a safe window.

7. Can I go in the sun after microneedling?

Sun exposure should be avoided for at least one week following treatment, and diligent SPF use should continue for two weeks. Freshly treated skin is significantly more vulnerable to UV damage, and sun exposure during the healing phase can cause hyperpigmentation (dark spots), particularly in deeper skin tones.

Apply a broad-spectrum SPF 30 or higher every morning, reapply if you’re spending time outdoors, and skip the spray tans and tanning beds during this window. If you spend a lot of time in the sun, this is worth factoring into your scheduling—fall and winter tend to be ideal seasons to start or continue a microneedling series, though spring is also completely fine if you’re taking the proper precautions in the days that follow treatment.

8. How many sessions will I need?

For most skin concerns, we usually start with a series of three sessions spaced four to six weeks apart. Acne scarring and more significant texture concerns may benefit from additional sessions. Once you’ve completed an initial series, annual single-session boosters can help maintain the results.

At your consultation, we will assess your skin and discuss a realistic timeline based on your specific goals—whether that’s addressing longstanding scars or maintaining the results of skin that’s already in good shape.

9. Is microneedling safe for all skin tones?

Yes, and we often recommend microneedling for patients with Fitzpatrick skin types IV through VI who may not be ideal candidates for certain laser treatments. This is one of the treatment’s meaningful advantages. Because SkinPen works through controlled micro-injuries rather than heat or light energy, it carries minimal risk of post-inflammatory hyperpigmentation across skin tones. 

10. How does microneedling compare to laser resurfacing?

Both microneedling and laser stimulate collagen and improve skin texture, but they work differently and suit different patients. 

  • Laser resurfacing uses light energy to resurface the skin and is particularly effective for brown spots and more significant sun damage. However, it involves more downtime and isn’t ideal for all skin tones. 
  • Microneedling carries minimal pigmentation risk across skin tones, involves less recovery, and can be performed more frequently as part of a maintenance routine. 

For some patients, the two work well together: a microneedling series to build collagen and improve overall texture, followed by laser to address any residual pigmentation—with timing and sequencing determined during your consultation based on how your skin responds.
